Erythropoietin is a glycoprotein which stimulates red blood cell formation. It is produced mainly in the kidney of adult and in the liver of fetu.

EPOKINE is a recombinant human erythropoietin, type-α. It is a glycoprotein hormone which stimulates the division and differentiation of committed erythroid progenitors in the bone marrow.

 

EPOKINE has the same biological and immunological effects as endogenous erythropoietin and contains the identical amino acid sequence of isolated neutral erythropoietin.

What are the indications to use Epokine?
A.

Treatment of Anemia in Chronic Renal Failure.

  • EPOKINE is indicated in the treatment of anemia associated with chronic renal failure, patients on dialysis or not on dialysis.
  • EPOKINE is indicated to elevate or maintain the Red Blood Cell level and to decrease the need for transfusions.
Treatment of anemia in Cancer patients on Chemotherapy.

Autologous blood donation.

  • EPOKINE is indicated to elevate the Red Blood Cell level to donate autologous blood.
  • EPOKINE is also indicated to prevent from reducing of hemoglobin for patients scheduled to major surgery who are not able to participate in an autologous blood donation.

 

Q.
What is the dosage of Epokine?
A.
Chronic Renal Failure
  • EPOKINE is administered  intravenously at an initial dose of 50 units/kg for 1-2 minutes three times a week. It can be given by either IV or SC route for patients with CRF not on dialysis. The dose increase is dependent upon the initial response. The dose can be increased, if necessary, by 25 units/kg in 4-week period. If hemoglobin is increased more than 2g/dl at a dose of 50 units/kg, the frequency should be reduced to two times a week. To correct the anemia, the target concentration of Hemoglobin is 10g/dl (30% as to Hematocrit). When the anemia is corrected, EPOKINE is given as a maintenance dose of 25-50 units/kg two or three times a week.
  • The unit dose of EPOKINE should not exceed 200 units/kg and the frequency should not be more than three times a week.
Cancer patients on Chemotherapy
  • The recommended initial dose of EPOKINE is 150 units/kg through SC injection three times a week. If the response is unsatisfactory after 8 weeks of therapy, the dose can be increased up to 300 units/kg three times a week. If the Hematocrit exceeds 40%, the dose should be withheld until it falls to 36%. The dose of EPOKINE should be reduced by 25% when treatment is resumed or the dose is titrated to maintain the desired hematocrit. If the initial dose includes a very rapid hematocrit response, the dose should be reduced. The hematocrit should be monitored on a weekly basis until hematocrit becomes stable.
Patients to participate in autologous blood donation
  • Prior to major surgery, it is recommended to take autologous blood two times a week for three weeks. Based on previous studies, EPOKINE can be given intravenously at a dose of 150-300 units/kg, two times a week for three weeks to elevate the red blood cell level. The recommended maximum dose to promote eryhtropoiesis is 600 units/kg, two times a week for three weeks intravenously. All surgery patients being treated with EPOKINE should receive adequate iron supplementation throughout the course of therapy in order to support erythropoiesis and avoid depletion of iron stores.

What is Macroxam Patch?

What is this Non-Steroidal Anti-Inflammatory Drug (NSAID)? What is the difference with other medication like chili plaster, cream and gel?

Non- Steroidal Inflammatory Drug o NSAID, is a class of treatment that provides an analgestic effect on the pain. As a result, pain relief last longer than ordinary treatments. MACROXAM, with the NSAID formula PIROXICAM, is the first anti-arthritis treatment that delivers the drug through the skin and directly to the inflamed joint, thus avoiding long term adverse reactions and damage to the digestive system, liver and kidneys.

Why should I choose MACROXAM over other oral NSAIDs? Does Macroxam have side-effect?

Because Macroxam is transdermal patch that delivers the drug through the skin and directly to the source of inflammation, it has no direct gastro-intestinal contact and thus greatly minimize the long-term adverse reaction and damage to the digestive system, liver and kidneys (Plasma concentration of Macroxam is 50 time less that the oral form but its concentration on synovial fluid is comparably the same). Skin irritation may occur in patients with hypersensitive skin.

One Macroxam patch is P75.00, but this is good for three days. You can even take a bath with it! Isipin mo lang na kumpara sa ibang NSAIDs na may presyong P76.00 per day (price of 3 popular oral NSAIDs), di ba mas sulit ito dahil good for 3 days na, minimized pa ang systemic side effects!

So in effect, the use of the Macroxam patch is not only safer and more effective but also more affordable in the long run because on a cost for 3-day treatment, use of Macroxam is 65% cheaper than the cost of other oral NSAID on a two-day treatment.

Paano ba ginagamit ang Macroxam?

Is there a difference in pain relief between Macroxam and liniment patches?

Liniments or counter-irritants have been for many years been used to alleviate muscle pain. It works by taking your attention away from the pain; instead, it makes you conscious of the menthol or “init/lamig” sensation that the patch gives. This provides temporary pain relief and will not relieve inflammation ( which causes pain ). Macroxam uses Piroxicam, a drug which for years has been trusted by doctors to relieve pain and inflammation. Because it is also anti-inflammatory, it hastens the healing process of the body, allowing you to return to normal activities sooner.

Many executives have body aches either due to work related stress or because of weekend sport activities. Many do not want that “elderly” smell, frequently associated with liniments. Macroxam is Piroxicam contained in a patch, it contains no menthol, no methyl salicylate nor camphor all of which emit a medicated odor. Where can I place the patch? Can I place the patch in areas other than the knee?

The patch may be placed in any muscular area or joint which is in pain. Large muscle groups, commonly in pain, are the lower back and shoulder muscles. Macroxam may be placed directly onto the muscle in pain. As for joint areas, the knees, ankles and elbow are frequently afflicted with pain due to arthritis or sport activities. For these areas, the patch is applied at the sides of these joints to provide maximum adhesion. Macroxam will penetrate the skin and infiltrate the muscle and joint spaces to directly relieve your pain.

Is it safe for kids?
A.

Since the medicine will not pass through the blood stream and into the internal organs, Macroxam will not cause untoward systemic side effects such as hyperacidity and bleeding. It is safe for muscle and joint pain for children 15 years old and above. However, pain in children is usually caused by other medical conditions which must be determined by their pediatrician.

I have low back pain and I was diagnosed to have scoliosis. How many patches can I put at one time?
A.

Scoliosis is defined as abnormal lateral curvature of the spine. In this condition, muscle are often stretched on one side of the body; also, nerves in the area are impinged by the vertebra. The combination of the two conditions causes pain that is aching (“ngalay”) in character. Many also describe it as sharp and “electric” (“parang kinukuryente”) which radiate to the side of one leg. It is sufficient to place one patch on both sides of the low back to provide you with relief but if the painful area is large, you may place as much as 3 patches on the side which hurts more. Assuming the correct posture will help prevent further aggravation.

Q.
My husband is 67 years old and suffer from osteoarthritis and hyperacidity, is it safe for him to use Macroxam?
A.

Elderly patients with arthritis often taken pain killers as needed. Many take as much as 5-6 times a day, resulting in hyperacidity. There are many types of pain relievers, most commonly used are Mefenamic Acid, Piroxicam, Ibuprofen, Indomethacin and Naproxen. These are collectivity termed “non-steriodal anti-inflammatory drugs” or NSAID and when taken orally, pass through the stomach causing gastric irritation. Most likely, your husband acquired hyperacidity due to frequent use of oral pain killers. Intake of these drugs causes “NASAID Gastritis” or Drug Induced Gastritis. Macroxam may be placed directly at the arthritic area such as the knee. Since the medicine does not enter the stomach, there is no gastritis.
